Retreat Registration Privacy Notice
Who we are
Health & Longevity Coaching LLC, operating publicly as Health and Longevity Coaching, is responsible for the registration information described in this notice. For questions or requests concerning that information, email Serra@HealthLongevityCoaching.com.
Information provided during registration
When you request or register for the OSM Midlife Transitions Mini Retreat Series, we collect the name and email address you provide, your preferred language, your responses to the registration questions, and any dietary, accessibility, or communication arrangements you choose to request. We also keep the record of your terms and optional media choices, payment status, and the communications needed to administer your place. Please share only what is needed for an arrangement. Do not enter medical records, diagnoses, medication lists, or detailed health histories in the form.
How we use this information
We use registration information to respond to your request, assess availability, arrange participation, communicate about the six sessions, administer payment and cancellations, and maintain necessary transaction and consent records. Registering does not subscribe you to promotional emails. Occasional news and marketing communications require a separate optional choice.
Payment and service providers
Payment is handled through the payment method shown during checkout. The website, form, email, payment, and storage services used to run registration may process information on our behalf. We may also share the minimum information needed with people helping us provide the event, such as an appropriate dietary or accessibility arrangement. We do not provide your private reflections or personal story to Old School Maker Space for its own publicity.
Photographs, audio, and video
The registration terms checkbox does not authorize identifiable photography, audio recording, video recording, testimonials, or publication of your personal story. Separate, optional, initially unchecked choices apply to photographs and to audio or video. Declining either choice does not affect your eligibility or participation. Any permission you choose is explained in the linked Photography and Recording Notice; permission to use your name, quotations, or personal story requires separate specific agreement.
Group privacy
We ask participants to respect one another’s privacy. Because this is a group experience, we cannot guarantee that another participant will keep a disclosure confidential. You may choose how much to share and may reflect privately.
How long we keep information
We keep registration, transaction, communication, and consent records for the period reasonably needed to administer the event and meet applicable business, accounting, safety, dispute, and legal obligations. Optional publicity materials are handled under the separate Photography and Recording Notice and the permissions you select.
Your choices and contact
You may ask about information submitted through registration, request a correction or eligible deletion, or change an optional communication or media choice by emailing Serra@HealthLongevityCoaching.com. Some transaction or consent records may need to be retained where required or justified by applicable obligations. We will make reasonable efforts to stop future uses of media under our control after an effective withdrawal, as explained in the Photography and Recording Notice.
Changes
If this notice changes materially before you register, the current version will be shown to you with the registration form. The version you acknowledged and the date of that choice should be recorded.
